People often arrive at the BMV searching for an exact form number, “BMV 3774,” “BMV 4809,” “BMV 5736”, because that’s what a clerk, a dealer, or a letter told them to bring. This page is a plain-language index of the most-used Ohio BMV forms: what each one is for, where to submit it, and which ones the Akron BMV (Ohio Deputy Registrar Agency 7731, 688 Wolf Ledges Parkway) can help you complete. The official, always-current PDFs live on the state’s forms library; we link to each one and add the context the state’s dense list leaves out.
Two offices, two form types. Some “BMV” forms are actually title forms filed with the County Clerk of Courts (titles are not issued by the BMV). Others are registration, plate, or license forms handled by a BMV deputy registrar like us. We flag which is which below so you don’t drive to the wrong office.

Most-used title forms (County Clerk of Courts)
Heads-up: these are title forms. Ohio vehicle titles are issued by the County Clerk of Courts Auto Title Division, not the BMV. File these there, then bring the resulting title to the Akron BMV to register and plate the vehicle.
